Veterans' Bonus Application Document Submission

INSTRUCTIONS for submitting supporting documents

INTRODUCTION

Thank you for your service.

This form allows you to submit additional attachments not included with your original application. 

If you submitted your application online, you received a confirmation email message containing an Application ID number. Please include that number in the space provided in the form below. 

If you lose your Application ID number please contact us at (617) 367-9333, extension 859 or reach out to our team by submitting a veterans' bonus inquiry.

If you submitted your application by mail, please provide the last four (4) digits of your Social Security Number in the space provided in the form below.

You must complete all of the required entries (indicated by a red asterisk *) in the form below.

Documentation and Supporting Materials


You are required to submit documentation confirming Massachusetts residency, proof of deployment(s), and proof of service. Your application will not be considered for bonus eligibility until all pertinent documentation is received. 

You can attach the documents below via file upload (this is the preferred method of submission) or submit via mail to:

For Active Duty Servicemembers, please submit

  • Commanding Officer Attestation Form (Commander's Letter)
  • Certificate of Residency
  • Proof of Service

For Discharged Servicemembers, please submit

  • Form DD214 (Discharge papers)
  • Certificate of Residency

For Servicemembers in the Reserves or National Guard, please submit

  • Certificate of residency for each DD214 activation period. Proof of residency must be within 6 months to 1 year prior to the activation start date.
  • Proof of Service
