MassEVIP Public Access Charging Program Application
This form is for applying for incentives from the Massachusetts Department of Environmental Protection's Massachusetts Electric Vehicle Incentive Program (MassEVIP).

If the answer to questions 1 or 2 is yes, or if the answer to questions 3, 4a, 5, 6, 7a, or 8 is no, the applicant is NOT eligible for funding through the PAC Program

Consistent with the terms of the VW Environmental Mitigation Trust, all documentation and records submitted in support of funded applications shall be made publicly available to the fullest extent provided by law.
The applicant is required to upload a copy of the most recent IRS W-9 form - Request for Taxpayer Identification Number and Certification, before submitting this application. This form must be signed and dated within the past year.
Complete and upload a W-9 form. This application may be saved and returned to later by clicking “Save and Resume Later” at the bottom of this form. However, any previously uploaded docs will not be saved and will need to be re-uploaded.

Is the applicant participating in the Eversource or National Grid EV charging station program?
In the attachment section please provide submitted application or, if utility program application has been approved, attach approval letter or equivalent.
MassDEP will not fund installation costs for projects funded through the National Grid or Eversource EV charging station programs.
Note: Applicants have 24 months to install charging stations at new construction locations and 6 months to install charging stations at existing location.
*“Government” means a State or local government agency (including a school district, municipality, city, county, special district, transit district, joint powers authority, or port authority, owning fleets purchased with government funds), and a tribal government or native village.

Be sure your coordinates are in decimal format, with at least 6 decimal places. There should be a “-“ at the front of your longitudinal (Y) coordinate. A local coordinate for example, would be (X) 42.275404, (Y) -71.303471.

I also hereby certify that I have authority to apply for MassDEP’s grant award. I am also aware that, in the event it is determined any grant funding was awarded based on false information, I or any organization on whose behalf I sign this Application, will be required to return the grant money.
